Parent Consent Forms*
Parent Consent Forms
Parental consent forms are available via the Parent Digital Consent System (PDC). Families can log into the PDC using their ParentVue credentials to complete consent forms assigned to their child. If you have a new ParentVue account, you must wait 24 hours after it is activated to access the PDC.
All families should review the list of required and optional consent forms for each of their children. Please complete the required Digital Consent Form and necessary optional forms by logging into the Parent Digital Consent System (PDC).
At Fairview Elementary, our teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used. Student Rights & Responsibilities*
Student Rights & Responsibilities
The 2024-2025 Student Rights and Responsibilities (found on the FCPS SR&R webpage) explains expectations for student behavior and adult responses to enhance school safety and to create a fair, equitable and supportive school environment (translated versions available). We ask that you use this document as a framework for discussing these expectations with your child or teen so that he or she is well prepared for success when the school year begins. You will also find that this publication includes information that you may need throughout the school year, including contact numbers and other resources.
All students in FCPS should review the Student Rights and Responsibilities.
- Review the Student Rights and Responsibilities and related resources on the FCPS SR&R webpage
- After reviewing the information, fill out the SR&R Acknowledgment Form
- The form must be filled out for each child/student in grades K-6 in the household digitally or on paper
- Parent/Guardians can acknowledge receipt of the SR&R in ParentVue in your child’s SIS account or sign the Parent Signature Sheet to acknowledge that you have received this document. Please e-sign through ParentVue or return the signed signature sheet to your child's school by September 30, 2024.

Pickup Patrol: Getting To and From School
Fairview uses a system called PickUp Patrol to help us track dismissal plans. PickUp Patrol (PUP), provides parents/guardians with a convenient way to make changes to a student’s dismissal plan and ensures that our students are safely dismissed to their correct after-school destinations.
DISMISSAL CHANGES: Submit changes to your child’s regular dismissal plans.
For example:
- Your child normally rides the bus but is getting picked up today.
- Your child is staying after for an activity.
How it works:
Your child's regular dismissal plan is entered into the system. This is called their Default Plan. Whenever you have a change to the Default Plan, you’ll submit it through the PickUp Patrol app. When the change is submitted, the school will be notified, and your child’s teacher will relay the change to them.
Getting started:
- REGISTER: You should have received a registration email with a link to set up your account.
- The web app is free to parents, and you can use it from your smartphone or computer.
- For easy access, follow the instructions for how to add it to your phone’s home screen (PUP is not accessed through the app stores).
- ENTER DEFAULT PLANS: Once you register, log into your PickUp Patrol account to confirm your child’s default dismissal plan. You must complete this step so that we can accurately dismiss your children.
Notes, emails or phone calls will be accepted for changes for the next few weeks, but starting August 31, all changes should be submitted through PickUp Patrol.
Bus
- Your ParentVUE account will show your child’s bus route number, the stop location, and estimated pick-up and drop-off times. With your ParentVUE account, you can activate the Here Comes the Bus app for tracking your child’s bus. You will need the FCPS school district code of 28982.
- You should arrive at your bus stop ten minutes before your scheduled pick-up time since schedules may vary slightly from day to day.
Kiss and Ride
- This year we are using name tags for Kiss and Ride which can be picked up in homeroom classes. When using Kiss & Ride, you must enter from Burke Centre Parkway and use Door 2.
Walkers
- Walkers enter at the playground, Door 5. All kindergarten students must be met at the door by the parent/guardian.
